My wife and I applied for her permanent residence application in mid-October of 2012, a day or two before her visitor visa extension was set to expire. We recently received news that they have received our application. I am Canadian and she is from Indonesia.

Today, my wife is tested positive for pregnancy but not doctor verified yet. We are in Alberta and she got an extension on her health coverage which will expire in mid-January.
Does anyone have an idea what to do first? Thank you in advance.

Make sure you have health coverage if delivery happens in Canada. In that case, the child will be covered, since it will hold Canadian citizenship. If the mother isn't insured, however, you'll get dinged for all expenses relating to her care.
Once the child is born, let CIC know about it - full disclosure is always advisable and it can only help your case.

Hopefully by the time the baby is born, the wife will have healthcare, because it will be impossible to buy private birth insurance now that she is pregnant.
