My fiancee's passport(Philippines) includes both parent's last names, but the mother's name is classified as a 'middle name'. Question 1 of the the personal details requires the Family name appear exactly as shown on the passport. My question is do I put both family names in this box or just the father's name?

Family name- her father’s name
Given names- her first name followed by her mother’s maiden name
