GCMS Notes for Parents application

Hi

If someone pays $5 for the GCMS notes application (where she has sponsored her parents and brother), does she need to pay $5 again for her mother & $5 for her brother?

So, should she pay $5 for each ($15 total) or $5 for the whole application?

They asked for the consent form of mother and brother too, though father was the principal applicant and she already sent his consent form. She is just confused whether she should pay again for her mother and brother separately.

it's $5 per request - not $5 per person

consent should be obtained from the principal applicant, spouse, adult children or any other person appearing in the records.
