Form IMM5669

I am an Express Entry applicant. I received a request letter for Form IMM5669e. However, in the request letter they have attached Form eIMM5669e (which is the same form but for refugees with electronic signature).
Someone in the group suggested me to submit the form which is attached via link in the request letter. Hence I submitted Form eIMM5669e.
Till now I have already submitted this form for myself and my wife. However, they have asked me for it the 3rd time. I am totally confused.

Call IRCC and check if they received the document. They can clarify why it was requested again. You can ask them as to which is the correct form to submit too.

Please also send the document via webform, to be sure it gets attached to your file.

I found a contact number but it mentioned that it can be dialed only from Canada. And I couldn't find the webform.

The number to call IRCC : 1-8882422100. If you are outside Canada, you may need to use some VOIP phone apps to call the number.
