Hey Guys i just have a a question pls do i need to fill form IMM5540 with application does my wife who is the sponsor need to fill this too with form IMM5481, pls help guys!
 
What country are you applying from? It will usually advise you in your country specific guide.
 
Okay. Then you need to follow this checklist:

The checklist needs to be included into your application as well. It tells you what forms are needed and who fills them out :)

By the looks of the checklist, your wife has to fill out IMM5481
