Filling up Adult Form: Work and Education History

Hi Everyone. A very quick question about how to put in the dates in Work and Education History.

For example in June 2013, 1st week I was employed, 2nd and 3rd week unemployed, and the 4th week employed again with a different employer. How do I represent this in the form since the dates asked are only YYYY-MM, and no days? Should i put something like this:

From (YYYY-MM) To (YYYY-MM) Work/Education
2013-06 2013-06 Work
2013-06 2013-06 Unemployed
2013-06 2013-06 Work

Do you mean that you had 2weeks off of transition from your old work to the new one? you can just omit the unemployed weeks.
From To Work
Whenever 06-2013 Previous work
6-2013 Present Present work

If you read the form carefully, it says that if you were unemployed, put the dates in the Work/Education field and provide reason for unemployment.
You can reference this for your knowledge and write starting with the recent one:

From (YYYY-MM) To (YYYY-MM) Work/Education Employer or School Attended
2013-06 2013-06 Work Employer Name (4th week employer)
2013-06 2013-06 2013-06-xx to 2013-06-xx Unemployed because of reason xxxx
2013-06 2013-06 Work Employer Name (1st week employer)
