Filling out form IMM5802 - title of LMIA exemption

[Nutshell]

I have been nominated through BC PNP and my boss is in the process of filling in form IMM5802 (Offer of Employment to a Foreign National Exempt from a Labour Market Impact Assessment) but she's not sure what to put in box 21 "Title of LMIA exemption being requested".

Please help! I want to go to the border tomorrow and this is the last box we need to fill in.

It should be Canadian-PT Agreement - R(204) (C). Box 22 will be automatically filled out as T13. See attached link for more information about filling out a form IMM5802.
