Family information (IMM 5645) [PDF, 1.42 MB] form with my visitor visa application through ircc portal. The general information on ‘‘getting ready to apply’ says that

Submit with visa application.
ircc portal has very limited space for attaching docs and this form is too big. Appreciate guidance
