My spouse just got her work permit from the new program while she waits endlessly for her PR...
Unfortantely her passport expires in October, so they only made it valid until then. Our consultant want to charge us again for this. Is this extension straightforward. I can't even figure out which form to use.
Also, anyone had luck getting ohip with this work permit and no job (yet)?

A work permit should make her eligible for OHIP (after the 3 month waiting period) if her primary place of residence is in Ontario once she has a job in place. See http://www.health.gov.on.ca/en/public/publications/ohip/ohip_eligibility.aspx
Note that she may also qualify from your PR application. How far along are you (and inland vs outland)?

She needs a new passport first. CIC does not issue temporary residency status (visitor, worker or student) beyond expiry of a person's passport. Your consultant really should have known this.
Once she has a new passport, she can submit the same form as she submitted with the PR app, IMM5710, along with a cover letter explaining the situation. Note that as she is extending the work permit, she will have to pay the fees again.
We submitted in January, so we over a year to go for preliminary approval.
The OHIP eligibility guidelines, to me, are not clear. It seems to imply that full time work for 6 months is required.

As I read it, she needs to have a job that will be in place for at least 6 months, but doesn't need to have already have worked for 6 months:

It may be worth calling. If they got the revised passport info (even if they didn't process it) before the date they issued the OWP, I'd ask if they could please consider reissuing it.
