There should be some kind of contact information, so that CIC doesn't think you just made them up, but I wouldn't think that a phone number is required though it could be useful. I suspect that they have to follow court rules which say that a statement made under oath must be believed unless there is evidence to the contrary.

Not a very good endorsement for your law firm as you're giving completely wrong information. If you don't even know the basic rules, you should not be attempting to help people with immigration matters in a professional capacity.
