I tried to get Reference letter from my HR but they don't issue reference letter with roles and responsibilities.
One of my previous manager who is now in different group but in same company has agreed to provide reference letter on affidavit with a condition that he'll mention that he's my ex-manager and shifted to another group 1.5 years back.
Is it fine? My last promotion was 3 years back under my ex-manager. Do you think it can cause any issue?
Should I approach my current manager, which I don't want at this point?

Your former manager can as long as he still works for the company and the letter meets other cic requirement. You may have to support this letter with other documents such as:
Pay stubs or a salary account statement as proof of salary payment
Promotion letter
Appointment/offer/confirmation letter.
Document to prove that your HR was not willing to write out a reference letter for you, e.g print out of email correspondence, etc
Is your former boss providing the reference letter on company's letter-head
