I have concerns about my upfront eMedical given to me by the clinic:

1) The report only lists 501 Medical Examination required. However I did all 4 (501, 502, 707, 712).
2) My eMedical shows the visit date incorrectly. It shows that visit date is 4th of October and that the report was issued on 6th of October. However I went and did all my tests on October 10th.

Should I upload this eMedical or request the new one from the clinic?
