I've received a job offer and the company are willing to support a dual intent LMIA.
Please, could someone clarify the process? Do they submit the LMIA forms and pay the $1000, and the temporary work permit application is included in that application, or do I have to separately apply for the work permit?

You should be asking your questions in the temporary foreign worker section of the forum. It's a good idea to search the forum first before asking questions. Often times your question has already been asked and answered repeatedly. Also check the cic website.
I went through this recently and yes one needs to apply for WP separately along with positive LMIA result. Given that yours is a dual LMIA, you may also apply for PR immediately with the same LMIA number.
