We are sending in her PR application in-land in the next few days. Her visitor visa expires February 28th. Do we need to renew her visa or does the application automatically extend her visitor status?

The application for OWP that you should also submit with inland app, will serve to give her implied status during the processing time. So no need to renew visitor status.
Note that implied status ends if she leaves Canada, so you are supposed to stay here for entire processing time.
You should file an open work permit application with the inland PR. That will give her implied status and eventually allow her to work after approval in principle. If you do not file an OWP application, you should file a visitor extension. If you do nothing, she will be out of status and have to rely on the policy not to remove otherwise eligible inland applicants who have an eligible sponsor.
Not only lose your implied status. Once your visa expires, you can't get back in to Canada once you leave. So once your visa expires, you are basically stuck in Canada until your PR is done.
So we just did the questionnaire to see if she is eligible for a work permit but she isn't. Does this questionnaire disregard someone who is applying but has not yet applied for permanent residency? Do I just submit the application anyways?

You send in the OWP application with the inland application. After stage 1, they actually process the OWP app and that's when you get the OWP. So you state very clearly on the OWP application form that it needs to be processed along with inland stage 1 process.
Since you are submitting the OWP with the app, you get immediate implied status. This is common practice for inland apps so CIC Vegreville will know what to do with it.
You need to print off the OWP extension app form, and submit in the same package as the inland application.
If you do it online, then CIC will not be able to link the OWP app to inland app, so OWP would just be refused upfront.
