Documents required & steps to request WES for IHM graduate?

Hi There,
I am a Hotel management graduate from IHm, Chandigarh(2013)- where degree is provided by IGNOU(B.Sc.- Hospitality and Hotel administration) and the exams are conducted by National council of Hotel Management and Catering technology (NCHMCT).
I want to get my WES evaluation done but for that I’m not sure what documents are required:
-Transcript from both(IGNOU & NCHMCT?
-What to mention in educational institute while registering for WES evaluation(whether to mention both separately or is it self explanatory to them?)- What to mention there(BSc from Ignou or Hotel management from NCHMCT)

Please help.
