documents for self employed doctor

I am a self employed doctor what documents should I provide to prove it .
Is an affidavit sufficient ?Please help.

No an affidavit won't be sufficient.

You will need to write a whole letter explaining what you do as a self-employed doctor (on your personal letter head), how long its been you doing your practice, the address & proof of your clinic, telephone bill (might help), your tax returns for the whole period you going to claim as experience, reference letters from the doctors you know.

These things should work for you. Correct me if I am wrong seniors.
