If a form does not apply to you on the document checklist, do you still check the box?
for example: the Use of Representative form. If I didnt use that form, do I still check in the box? or leave it blank? Should we check all the boxes off even if we didnt use those forms?

We hand wrote N/A for the forms that didn't apply for us.
