do I need both birth AND baptismal certificate?

hi again,
I am reading country specific guide (Poland), and I found one thing:

"Provide the following:
•national identity cards,
•birth certificates,
•baptismal certificates or,
•other official document confirming relationship for you and each of your family members"

So, I have national ID (I understand I have to copy and notarize it), I soon will have my birth certificate, but do I need baptismal one as well? I was baptized, don't care since many many years, so I don't have it, and I don't know how long it takes to get certificate. Also additional costs...

Can you guys advise me here - do I need it? Or National ID and Birth Cert. would be enough?

Note the use of the word "or" above. You are not required to submit all of those documents.

yes, i see OR, but it is after this baptismal certificate, so I wasn't sure how to interpret it
