I received my PR Card in 2009 and it expired in 2014. I have never lived in Canada but have visited a few times. I am not ready to move to Canada.
My question is -- do I need to surrender or do something to my old PR card and contact Canadian Government BEFORE I start a new application? Is that needed?

Yes - this is needed. Otherwise your new application for PR will be automatically refused.
It's not really about the PR card itself but the PR status. You would have to renounce that if you want to apply again. Here's how to renounce: http://www.cic.gc.ca/english/information/applications/voluntarily-renounce.asp
