Different names on educational certificat and passport

I only have the first name printed on my all educational documents.
But on my passport i have my full name printed.
E.g. John Smith
Educational certificates contains only John

in work permit application form thereis a question stating

have you used any other name ?
shall i answer it as YES and provide only my first name or shall i andwer it as no

Provide a supporting letter to explain all name mismatch in all records.
