Dependent children

Need some directions the visa office instruccions says the each dependent child, regardless of age has to fill out ADDITIONAL FAMILY INFORMATION (IMM 5406) form, when i send my App. to NS just send my wife´s and mine, is this new??
Please advice.

This form has to be completed separately by Principal Member , the spouse & the Dependent children over the age of 18years
It's clearly written in the VO Specific instructions.

So you have to fill that as specified in the instructions. My VO ( Pretoria) instructions clearly mention for dependent children over the age of 18.
