i am living with my husband in dubai and i m canadian citizen .

i know that from that guide i have to print residency questionnaire and print it by hand and attach it with my application ....my question is there are two decalaration pages as well , so do we have to signed that to ....or are they for afghan nationals only ....
also there are form asking travelling history , education history , military service ....so do i have to fill that up as well....
or do i just need to submit the residency questionnaire for abu dhabi
