I'm preparing my documents for my eAPR, and one of them is the declaration of non-accompanying parent. My ex and I divorced in 2012, and I have this form notarized from 2013. My question is, does this form expire? Do I need to get a new one? My ex lives overseas, and it would be hard to coordinate.

Grateful for any input.
