Alternatively, even the VFS will have a form to fill. You will have to fill by pen though. Electronically, you have flexibility to edit the table if you are falling short of space

Pls specify which category you are applying under and through VFS right? I will then let u know.
You are submitting the docs on your own so no representative form!

Under SPP, I am seeing many students submit an SOP in addition to the above documents.
Its not a highly recommended document for univ category in which i hv applied so cant tell u much. You may ask many students here, infact browse through the existing threads n ull find.
The person helped you secure admission but he isn't submitting docs on ur behalf so you dont need to submit the representative form.
