As for the Condition 51 thing, we got it, not that it matters to us at all, we live together whether it's written there or not, but yes it's still being given out.
If it's not on your wife's COPR, I wouldn't take it as an error. Them not putting it is an arguable point that it doesn't apply to your situation IMO.
Yeah, I dunno. It's definitely supposed to be there. My wife and I were only married 3 months when we applied and we don't have children together. Furthermore as I stated above, another member I speak to regularly who also went through SVO and had visa issued on the same exact day has the condition and we don't.

Does Condition 51 absolutely apply to anyone who falls within the criteria? Or is it more case-to-case? As in if your application overall is very strong, they might say there's no need to apply the condition?

As far as I know anyone who doesn't have children with their sponsor spouse and is married and for less than 2 years when they apply should have the condition.

I'm sure it will as the repeal when it happens will be inclusive of current PR's living with the condition currently. So there really is no need for them to keep adding it to COPR anymore. I just think it's extremely odd that the same visa office issues it on one visa and not another one on the same exact day.
