we are applying on the family visa and on the checklist it asks about the medical exam however it says that you only need to do if they request it. So do you tick it or leave it blank on the checklist?

It's up to you. There will end up being a lot of boxes on the checklist that don't apply to you for various reasons (stuff about dependent children, etc) depending on your situation. Some people check them anyway, some people put an "X" or write N/A, it doesn't really matter, do what you want.
