Changing custodian after CAQ

When applying for a study permit for a minor in Quebec one needs to present a Quebec Declaration of custody of a child to receive an CAQ. After that the federal government is asking to fill out their own IMM5646 custodianship declaration. Can one use a different custodian on the federal form? Thanks.

Can anything be done if parents lost trust in the custodian they originally assigned and need to make a change?
