I got nominated to sponsor my parents last year. I submitted the application in November 2022 and I received the file number in July 2023 and awaiting the next step. However, my dad (principal applicant) has to undergo an open heart surgery and may not be able to travel. I also heard that his medical test might get rejected due to his health conditions. Having said these, I am considering assigning my mom as the main applicant and my dad as the dependent. I am wondering if it is doable and if not, what the other options are.

I think OP can change the applicant. I have seen some other members did it by updating all the forms. That's usually the case if one of the parents want to land first and the other wanted to wait. Since PA must land first, that's why those members wanted to switch PA.
BUT the issue with OP isn't about who is the PA. BOTH parents need to pass medical before the sponsorship application for the parents can be approved.
If one of the parents failed medical, neither of them can become PR. (application can be rejected if one of the dependents failed any of the checks)

Both your mother and father must pass the medical in order for the application to be approved. This will be true regardless of who the primary applicant is. If your father cannot pass the medical then this means your mother will be refused too.
