The medical request forms i recieved has a missing name for my daughter in the Given Name box instead her name is printed in the Family Name box and the family name is missing altogether.My question is should i proceed with her medicals on the same form...?
Also she has a new passport ....with an added surname which was not mentioned on the old passport when application was filed.

She can go for meds on the same forms but you need to mention it to your DMP & he/she can correct it on the meds forms (my DMP did the same for me).You also need to submit her corrected name passport copy both to DMP and to London office as well ,accompanied by a cover letter explaining the whole issue and requesting for a correction in their records.In the cover letter also do mention the difference in name of your daughter in her new passport.
