CEC Inland Dec 2018 AOR - IMM 5669 Required

Hi,

I got an email from CIC yesterday asking for IMM 5669.

Here is the message. Do you guys know where to upload this. Did anyone get a similar message like this.

Please help!



Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please be sure to include your parents' information.This must be received at this office by: 2019/02/15.

Use the Cse form and be sure to include your file number and uci. In the little comment box, mention that you've been asked to upload said form.

I just found out there is an actual place to upload requested documents inside your express entry profile
