Hi! I can't find any related topic regarding this matter. I would like to ask, what if we cannot complete to fill-up the immunization form? Is it okay if our doctor only filled out all the vaccines that have been vaccinated to us and those vaccines that are not been vaccinated were left blank?

I hope anyone can answer this. I am so confused because my siblings are not able to take other vaccine shots when they are toddlers and the doctors said they are no longer needed to be injected.

Your doctor can only write what immunizations your children have received. If he states that your children received an immunization when he or she has not then that is a false declaration on the part of the doctor.
If there are immunization shots that your children need to receive then have them have the shots and then get the nurse or doctor to fill the form.
There are some immunizations like Rotavirus, H Influenza Bexsero which are primarily for younger children. And if the child has grown up already and is in their teens then there is no point having these shots.
You can ask your doctor to fill in "not required at current age" and have him or her sign the form.
Questions for these types of forms are usually directed to the doctor if they arise. The doctor needs to say that you have the required shots and can also say you no longer need certain shots.

Thank you for the explanation. My concern is my wife's flight is around the corner (2-3 weeks) and the Immunization Form is not yet completed and we're afraid she can't complete it on time for their flight. And one more thing will they be questioned or barred from entering Canada once the immigration see it's not yet complete?

2 weeks is plenty of time to fill up such a form in my opinion.
I have no idea how border officials will deal with this just based on the immunization form. But I am guessing that they have passed their medical exams and are deemed healthy. You should be able to pass into Canada and then go see a Dr in Canada to complete the immunization form
