Canadian Citizen card - Changes to family name followeing divorce

Hi,

I had received my Canadian Citizenship card under first marriage family name ABC, I have since then remarried and would like to change the family name on my Canadian Citizenship card to DEF.

How do I proceed for this change?

Presently my passport, SAAQ (driving license in Québec) and RAMQ (Medical benefits) cards are both under my remarried name (DEF).
