Canadian applying for US I-131 Re-Entry Permit - it wants a copy of visa page?

USCIS requires several docs with the I-131 for PR's applying for a re entry permit. One of them is:

"...a copy of the visa page showing your initial admission as a lawful permanent resident"

What is this? There was no special stamp or visa page on my Canadian wife's passport when she entered the US firstly as an LPR. Please help!

It also asks for "A copy of the Form I-797, Notice of Action, approval notice of your application to replace your Permanent Resident Card (also known as a Green Card or a Form I-551)"....are we supposed to have kept this approval document?
