You can split the information across multiple documents. Your old passport + Health card. They actually will ask if you have a Canadian ID if you only list your home country passport and take Health card as a valid one and ask you to add to your application form.

Can we use health card as a proof of identity for passport application?
My Driving licence is expired, and other proof i have is my old country passport.
Reason i am asking is because Health Card isnt listed here

I applied last week for the passport and presented my Driver's License and Ontario Health Card as ID's (The lady at the passport counter took the original and scan it to their system).

I'd say that a health card is an ID card issued by the provincial government.
