I have received the medical instructions from NDVO on Nov 8, 2012. Currently I'm in Canada and CHC has my Canadian address in the file. I completed my medicals on Dec 10, 2012 in Canada itself. Later I noticed that the instructions were for India, Bhutan and Nepal and not for other country. I have not informed CIC that I'm doing medicals in Canada. DMP completed my medicals by filling necessary forms and have given me a copy of SECTION A that I have sent to CHC. My questions are below.
1. Should i inform CIC that I have completed my medicals in Canada?
2. Is there any problem to complete the medicals with the instructions that they have sent now? ( India, Bhutan and Nepal)
3. Do I need to call CHC and ask about this?
then why they have written that if you are doing the medicals in any other country other than India, Bhutan and Nepal, we need to inform CHC so that they have send different instructions? DMP told me that the medical reports will be going to Ottawa? Do I need to ask DMP to send the medical reports to NDVO as my visa office is NDVO?

No problems at all. You can do your Meds from anywhere as long as they are done by authorized DMPs.

Earlier there was no requirement for separate instructions if Meds were done in country other than India, Bhutan, Nepal.....I think this is a new obligation.
As you have already sent the Section A to CHC, you have in a way informed NDVO that your Meds were done in Canada. Your DMP will not follow what you ask him to do as he is bound to follow the instructions he has from CIC.
