Where are you from? I'm trying to answer your questions
1) Have you checked your letter well? Generally you need to do your medical in 30 days time from the date of issue of the letter (and not the date you received the letter or the date your ecas changed).
2) Better to follow the time line they have provided. They want the applicant to follow the same, that's why they have mentioned it.
3) In that case please communicate the same to CHC without losing any time and ask for their suggestion. Don't forget to put your case number on the letter/email and on the envelope (in case of letters).
