Biodata missing in imm 1017 E medical request 2018

Hello i receive a medical request from my pr application but the biodata of the imm 1017 E is incomplete, no IME number wich i need in order to make a appointment with the panel psysician, can anyone help me with information

I think you should use the newest Acrobat reader. You will have a complete form.

I have the latest adobe to, what i did is i contact cic using the web form and they send me the medical request again, but this time the biodata is good

Hi! I'm having a problem with my form as well and have no luck in contacting CIC through the WebForm. I may be doing something wrong :|
Can you please help me? If you can tell me the steps you made, that would be truly appreciated. I need them to send a revised medical request. THANK YOU.
