Does the biodata form refer to form IMM 1017?

If so, it comes in my case as attachment to the medical request in the email.

I brought one paper copy to the doctor during the medical exam. Is it sent by the doctor, or shall I get it back and send it by myself?

Bio data form will come with RPRF and most often, separate from MR. However, there have been individuals that have received all three together...MR, RPRF, and Bio data form. This is something YOU fill out and send back to CPP-O (follow the instructions in the email). This is a step after eligibility assessment has been completed.

The best advice I can give you is...follow the instructions in the email you received. For MR, you see a Doc. For RPRF, you pay and send confirmation to CIC. For Bio data, you fill out and send back.

Second that! Biodata form has to be filled out by you, scanned and emailed back to CIC.

Ya. Toby's done good for you guys with his filable form...just make sure you keep checking the validity of the form. CIC doesnt inform when they change it, rather just slap a version # on the bottom and off it goes. I've seen people's documents returned cuz they used an older version. Doubt they will be changing the Bio Data one anytime soon, but you never know.
