Can you please some one tell me about the background declaration form?
In education history they are asking about the question of
certificate and diploma issued.
What i have to write down in this question?

So if you received a diploma you write that and what it was for. For example if you graduated as a social worker you write diploma - social service worker

Certificate you do the same and what it is for - sometimes trade schools give these

Actually its for my wife.and she studied dentist.
And its also more column in this list.we have to write down the all the information from school to college.
So what i have to write for certificate and diploma issued for each column?
