No, you have to create a separate account for your MIL and apply from that account. You may also need to create a new email ID for your MIL (if she doesn't have one) before you can open an new IRCC account for her.
Once you are able to open a new IRCC account for your MIL, you can basically upload all documents on her behalf and pay fees from Canada using a credit card after logging into her account.
I do not personally know the latest version of this form but general advice with IRCC is to always use the most current official version from IRCC website given IRCC have a habit of rejecting applications on old versions.
Just use the latest one. You will get a list of documents that need to be submitted with your application (after your online questionnaire) . Click on on the link and you will be taken to the latest available form.
