Applying for Bridge Work Permit

Hi

I am applying for bridge work permit and I have given medicals recently and I answered YES to question "Medical exam performed by CIC Panel physician in last 12 months and in required documents "Proof of Medical Exam" is needed.When I am done with medicals the doctor gave me invoice but no specific form(IMM 1017B).

Can I upload the Medicals request PDF(IMM 1017E) I recieved from CIC in that place.Please let me know what I have to upload there.Help me out guys

What does your document checklist say when you login to mycic? does it ask you to upload medical results? if not, then you don't need to.

yes it is asking
This is how it looks in my account because I answered yes to question asking me have I done medical exam in last 12 months

But I am confused what document to upload

One option is you can go back and change your answer to no. Then your document checklist will not ask for it. But if you are working in a field where medical exam is required, then I am not sure. Some one else may be able to answer this.

Upload the receipt provided by the doctor (the one with the IME number on it).

The document I have has doctors information on Invoice and IME number included in Invoice and Description says "IMMIGRATION MEDICAL" and price of $180.

Is that the one I have to Upload?

I would like to know whether its a big issue if I upload wrong document and is it OK to go ahead with the one I have
