I am a caregiver applicant. I received my LMO last December and my gathered documents will be picked up this January. Please help me regarding the following:

No. 9: Country where applying - status I selected other and filled up the space provided applying for work visa... I am thinking of putting visa instead.

Employment:

No. 1 - not working
Previous employment:
I have 7 previous employment
I add another page of the same section
City/town _ Pintak, Tel-Aviv, Israel? Is it right?
for the contract of my 7 previous employers I have only my first employer and the rest is stamped visa/ what shall I do with it?
or anyone here who can give me tips in filling up the form? I need help...
