Go to the 4th floor, office of the fingerprint Bureau, CID, Telangana Hyd (Located in Nampally, Telangana State Police Headquarters). They will ask for Aadhaar card, passport and Indian Police Clearance Certificate photocopies and originals. Please make sure to bring the FBI form FD-258. Just follow the instructions and you can be out within 1-2 hours.

Also, as you mentioned indian pcc is MUST. I was sent back as I only had a copy and not the original with me. They said e-Adhaar works instead of physical Adhaar card. And yeah, you will need the FBI form printed on a normal piece of paper (a4 size works) or on a cardstock (not necessary).
