because I am also in similar situation and need to get affdavit for one and the same name but I don't remember that I have selected yes while being asked.

This is in the first form when we enter the Name. I am talking about post ITA forms.

I think in your case name is not change its incomplete mentioned in few documents.
I think in LOE you can mention about it or if need to be on a safer end get a affidavit done thats both are one and the same person and also explain in your LOE about it..
