ADR for Schedule A. Which form to use - IMM5669e or eIMM5669e?

Hi,
I received ADR for Schedule A. The mail says this:
 Schedule A: Please be advised that gaps have been identified in section 8 of your Schedule A form, from 2013/06/15-2019/10/10. Please resubmit your Schedule A form, ensuring that there are no gaps in the timelines. This must be received at this office within 7 days.

Section 8 in IMM5669e and eIMM5669e are different. Also, in my application, I only have to upload against my name and not on my spouse. Can someone help me identify which form I need to fill and which section they might have mentioned?
