Address you lived

I completing spouse sponsor application. In one of the question asking sponsor if you have used other name like nickname etc. The sponsor got married and her last name is her husband last name now should she put there her previous last name? Or say No.

Second question is List all address you lived other then Canada. She was about 12 when she came to Canada from Pakistan and she was living there with her parent that was about 9 years ago. So should she say No I did not live other than Canada or put Pakistan address?

She should include her previous last name.
She should also list her address that she lived at in Pakistan.
