address history post ita

Hello everyone! i seek your help in filling out details on my application.
How do i fill out my address history in the application? It asks for P O Box no.... Can i put in my house no. there? And also Street no. ans street name are mandatory in few places... dont have a street no. What do i do? And also can i leave those columns blank where its written in the bracket (if applicable) or should i write N/A everywhere instead of leaving any column blank?
