additional sheet to background information form 5669

Hi experts,
question number 8, 9 and 12 the spaces given is not enough to write with accurate information.
Hence i decided to add extra sheet for this fields.

my question is :

1. should i fill up the questions in the form as much as i can and attache an extra sheet will full information

or

2. i should leave the questions blank on the form and simply attach extra sheet.


i am afraid if somehow they lost my extra sheet option 1 will support me.

or i am thinking of third option:

3. in the form write : please check the sheet attached. and then attach the extra sheet.

which option is the most wise one to pick ?

I would go with option 1. Create a similar format in xls and then use it.
