Quick question just to be sure...we are finalizing all documents to mail out soon.

For additional note on the separate sheet of paper, do you know if you supposed to sign (print name) or just need to be attached (staple) on the actual form ?
 
Don't staple anything. Use paperclips or just put the additional paper behind the form. There is no need to sign it.
 
On any additional papers make sure you have the sponsors name, applicants name and date of birth. If it gets misplaced it will be easy for them to locate the file . Do not use staples paper clips are fine.
