On the form Additional Family Information (IMM 5406), am I supposed to list myself (I am the Canadian citizen sponser) as the "spouse"? I can't tell if it's only for when a sponsored spouse is applying for PR as well, or if it's supposed to include me..
Also, am I supposed to put her (the applicant)'s mother and father on this form? (they are not being sponsored for PR)
Yes you include yourself (sponsor) as the spouse. And you list her mother and father, her kids (if any) and any brothers, sisters, half sisters, half brothers.
